I hereby give my son/daughter permission to attend the “Jr. High Mission Trip” to Duluth, MN sponsored by Hawley Lutheran Church. I understand the possible dangers and risks involved with a Mission Trip and absolve Hawley Lutheran Church from any injuries and/or death sustained by my child/youth while they are participating in the Mission Trip. In the event of an emergency I authorize the trip leader to make any necessary medical decisions for my son/daughter.
